You keep hearing acne is hormonal. Or genetic. Or something you have to wait out. In my practice, the picture looks very different. Most of the breakouts I trace back start in the gut, and eight ordinary foods keep showing up as the culprits. Not the junk food you would guess. Foods you probably ate this week thinking they were fine.
1. Ultra-Processed Foods
Ultra-processed foods are packaged products built from industrial ingredients you would never find in a home kitchen: emulsifiers, refined oils, protein isolates, sweeteners, gums. Think seed oil chips, hot dogs, packaged lunch meats, boxed cereals, most protein bars, and diet sodas. They damage the gut long before they ever hit your bloodstream.
The emulsifiers are the quiet villains. Chassaing et al. (2015, Nature) showed that carboxymethylcellulose and polysorbate 80, ingredients in half the aisles at any grocery store, thin the mucus layer that lines your colon. That mucus is not decoration. It is the buffer between your gut bacteria and the wall of your intestine.
Thin the buffer, and bacteria start touching the wall directly. Tight junctions loosen. Zonulin rises. LPS leaks into circulation and puts your immune system on quiet alert. That alert shows up on your face as insulin resistance climbs, IGF-1 signaling amplifies, and your sebocytes pump more oil into pores that are already clogged.
If your grocery cart has more barcodes than produce stickers, this is the first place to look. I have watched clients cut ultra-processed food for four weeks and clear cysts they had for years, without changing a single skincare product.
2. Refined Sugar
Refined sugar is the white stuff, the syrup, the granola sweetened with cane juice, the oat milk latte, the trail mix. Anything that spikes blood glucose hard and fast counts. It is the single most direct dietary driver of acne we have on paper, and the mechanism is not gentle.
Smith et al. (2007, American Journal of Clinical Nutrition) put men on a low glycemic load diet for twelve weeks and watched acne lesions drop. The chain reaction is simple. Sugar spikes insulin. Insulin spikes IGF-1. IGF-1 lights up mTORC1 in your sebocytes and drives both sebum production and follicular hyperkeratinization, the exact two things that build a clogged pore.
Sugar also feeds the wrong bacteria. It thins out your short-chain fatty acid producers, weakens tight junction proteins, and raises zonulin. Same leaky gut story, different food. The LPS that slips through amplifies inflammation through TLR4 all the way to your skin.

3. Dairy
Dairy is not one food. It is milk, and milk carries two proteins acne-prone skin does not love: whey and casein. Skim is the worst offender, not whole. Yogurt and hard cheese hit people differently, but any A1 dairy is worth questioning if you break out.
Adebamowo et al. (2005, Journal of the American Academy of Dermatology) surveyed teenage girls and found the highest milk drinkers had significantly more severe acne than low-consumers, with skim milk showing the strongest association. Follow-up work in older women found the same pattern.
Two mechanisms run at once. First, whey and casein spike insulin harder than the sugar content alone predicts, and elevate IGF-1 for hours after a glass. That is the sebum loop again. Second, A1 beta-casein breaks down into a peptide called BCM-7, which slows gut transit, disrupts mucin, and can leave tight junctions inflamed and loose.
When I ask a client to run a strict thirty day dairy trial, this is the group with the fastest visible change. Not always. But often enough that it is my first pull.
4. Gluten
Gluten is a protein family in wheat, barley, and rye. The gliadin fraction is the piece that matters here, and you do not need celiac disease for it to affect your gut. That is the whole point of the newer research.
Hollon et al. (2015, Nutrients) tested intestinal tissue from celiac patients, gluten-sensitive people, and healthy controls. Gliadin caused measurable zonulin release and increased intestinal permeability in every single group. Not just the celiacs. Everyone.
Zonulin is your gut barrier's release valve. Gliadin binds a receptor called CXCR3, triggers zonulin, and pries apart the tight junction proteins between your gut cells. LPS and undigested peptides slip through, systemic inflammation ticks up, and IGF-1 and androgen signaling amplify on your sebaceous glands.
I do not tell every client to cut gluten forever. Some tolerate sourdough or ancient grains just fine. But if you are breaking out and eating wheat three times a day, a clean four week pull is one of the cheapest experiments you can run. Pair it with a targeted gut cleanse and the barrier starts resealing while the trigger is out of the diet.
5. Artificial Sweeteners
Artificial sweeteners are the ones marketed as the smart swap. Sucralose in the yellow packet. Aspartame in diet soda. Saccharin in pink. Plus the newer stevia blends that hide erythritol and sucralose in the fine print. None of them are neutral in your gut.
Suez et al. (2014, Nature) showed non-caloric sweeteners shift the composition of the gut microbiome, crashing the populations that produce butyrate. Butyrate is the short-chain fatty acid that feeds your colon cells and glues your tight junctions together. Less butyrate means a weaker barrier, higher zonulin, and more LPS moving into circulation.
That LPS load fires TLR4 signaling, drives IGF-1-linked sebocyte activity, and raises cortisol-driven sebum production. The diet soda is not calorie-free at your skin.
If you are using sweeteners to dodge sugar, I hear you. But the fix is not swapping the trigger. It is retraining your palate off sweet altogether, and rebuilding the dysbiosis the sugar and the sweeteners both left behind. That rebuild does not happen in three days, and anyone selling you that is lying.
6. Seed Oils
Seed oils are the industrial oils extracted with heat and chemical solvents from corn, soy, sunflower, safflower, cottonseed, grapeseed, and canola. They are cheap, shelf-stable, and inflammatory. They are also in almost every restaurant meal and every packaged snack you touch.
Simopoulos (2002, Biomedicine and Pharmacotherapy) documented what happens when the omega-6 to omega-3 ratio in the modern diet climbs to 20:1 instead of the ancestral 1:1. Excess linoleic acid converts to arachidonic acid, which feeds pro-inflammatory eicosanoids like PGE2 and LTB4. Systemic inflammation climbs, tight junctions loosen, zonulin rises.
At the skin, arachidonic acid enriches your sebum with linoleic acid metabolites that oxidize, feed comedogenesis, and give Cutibacterium acnes something to work with. Your pore chemistry literally changes based on the fat you cook with.
Switch to grass-fed tallow, ghee, extra virgin olive oil, and coconut for a month and watch the skin calm. If you also need to help the barrier catch up from the outside while the inside repairs, a barrier-repairing tallow cream is what I reach for.
7. Alcohol
Alcohol is not a moral issue. It is a chemical one. Ethanol and its first metabolite, acetaldehyde, are direct gut irritants. Even moderate drinking damages the intestinal barrier before you feel a single thing the next morning.
Rao (2009, Hepatology) mapped the mechanism cleanly. Ethanol disrupts tight junction proteins, occludin and ZO-1, through oxidative stress and MLCK activation. Bacteria and their LPS translocate across the gut wall into portal circulation. TLR4 lights up. TNF-alpha and IL-6 climb. Cortisol elevates. Your sebaceous glands respond.
The dehydration piece is real, but it is downstream. The barrier damage is the primary event, and it happens with two glasses of wine on a Friday night. If your skin cycles between clear and cystic and you cannot figure out the pattern, this is a legitimate suspect.
I do not ask clients to become sober. I ask them to run four dry weeks and watch what happens. That data is usually louder than anything I could say.
8. Excess Caffeine
Coffee is not the enemy. Excess caffeine, especially after noon, is a stress hormone amplifier. Lovallo et al. (2006, Pharmacology Biochemistry and Behavior) showed habitual caffeine keeps cortisol elevated across the day and blunts the nighttime cortisol dip your body needs to repair.
Cortisol is a direct sebum driver. It raises androgen sensitivity in your sebocytes, upregulates cutaneous CRH receptors, and primes your skin for neurogenic inflammation. On top of that, caffeine suppresses the slow-wave sleep window where your skin barrier does its overnight repair. You lose the healing time exactly when the trigger is still active.
If you drink three cups of coffee, an evening latte, and a pre-workout, this alone can keep you breaking out no matter how clean your food is. My rule with clients: last caffeine by noon. If that sounds impossible, that is your answer.

9. What to Eat Instead
Cutting is only half the work. You have to feed the microbiome and the skin what they lost. This is the part most articles skip, and it is where the visible results actually come from.
The basics I put on every client's plate:
- Wild fish, pasture-raised eggs, grass-fed red meat, and organ meats for bioavailable retinol, zinc, and B12
- Cooked non-starchy vegetables and fermented foods like sauerkraut and kimchi for prebiotic fiber and postbiotic acids
- Bone broth, slow-cooked collagen-rich cuts, and gelatin for glycine and proline
- Berries, herbs, extra virgin olive oil, and avocado for polyphenols and monounsaturated fat
